You are the O&G BST1 covering the labour and postnatal wards overnight. A midwife calls you urgently to a 32-year-old woman who gave birth 40 minutes ago by spontaneous vaginal delivery at term, following an induction of labour.

She has soaked through a pad and there is a steady trickle of fresh blood. She is pale and her heart rate is rising.

How would you approach this?

Question

How would you approach this patient in the first five minutes?

Example answer

This is a primary postpartum haemorrhage, and the priority is simultaneous resuscitation, finding the cause and calling for help.

Call for help early: the midwife in charge, the Registrar on call, and a second midwife.
An A to E assessment with the woman flat, oxygen if she is compromised, and continuous monitoring.
Two large-bore cannulae, bloods, and warmed crystalloid while I assess.
Quantify the loss by weighing pads rather than estimating it visually.
Rub up a contraction, empty the bladder with a catheter, start an oxytocin infusion and give tranexamic acid 1g intravenously.

Question

What examination findings would you look for, and what are the likely causes?

Example answer

I would work through the four Ts, the framework used in the national postpartum haemorrhage guideline.

Tone (the commonest cause): palpate the fundus; a soft, high uterus suggests atony, and her risk factors here are induction of labour and grand multiparity.
Trauma: inspect the perineum, vagina and cervix in good light with adequate analgesia, looking for tears, an extending episiotomy or a haematoma.
Tissue: check the placenta and membranes are complete, as retained products prevent effective contraction.
Thrombin: look for oozing from cannula sites and ask about anticoagulants, abruption, sepsis or pre-eclampsia.

What investigations would you organise, and what would you ask the laboratory for?

Example answer

Bloods, urgently and repeated: full blood count, coagulation including fibrinogen, urea and electrolytes, and a venous blood gas for a rapid haemoglobin and lactate.
Group and crossmatch at least four units, and confirm whether a valid group and screen sample is already in the blood bank.
Continuous pulse, blood pressure, oxygen saturations and hourly urine output via catheter, escalating through IMEWS and documenting every trigger and response.
Call the blood bank directly to say this is an obstetric haemorrhage so they can prepare products without waiting for a form; a fibrinogen below 2g/L predicts a severe course.

Question

She has now lost 1,500 mL, remains tachycardic, and the uterus is still soft despite an oxytocin infusion. What is your management now?

Example answer

This is a major obstetric haemorrhage, and I would say so clearly and activate the protocol.

Ask the Registrar to attend at the bedside immediately, and inform the Consultant on call, the anaesthetic team and theatre.
Activate the major obstetric haemorrhage protocol, with a dedicated person communicating with blood bank and recording events.
Uterotonics in sequence: ergometrine intramuscularly (avoided if hypertensive), carboprost intramuscularly (avoided in asthma), and misoprostol.
Bimanual uterine compression while awaiting drug effect, and transfuse packed red cells while correcting coagulopathy.

Question

Bleeding continues despite all uterotonics and bimanual compression. What happens next, and what is your role as the BST1?

Example answer

She needs to be in theatre urgently. My role is to maintain resuscitation and support the senior team.

In theatre: examination under anaesthesia to exclude retained tissue and assess trauma, then intrauterine balloon tamponade as the usual next step.
If bleeding persists: compression sutures, stepwise uterine devascularisation, interventional radiology if available, and peripartum hysterectomy as the definitive option.
My role: maintain accurate records, chase blood products, communicate results, hand over clearly to the anaesthetic and theatre teams, and ensure the Consultant is present or directly informed.
Afterwards, arrange a high-dependency bed, debrief the woman and her partner, and complete documentation and an incident report.
